First Step: IRB Inquiry Form
Please complete the IRB Inquiry Form. This form should be completed when a researcher, faculty/staff member, student, or community member:
- Is unsure whether their activities/study require IRB review and confirmation is needed
- Thinks that their study does not require IRB review, but confirmation is needed
- Knows that their study requires IRB review, and they would like to notify the IRB of their intent to apply for IRB approval
- Has a different question for the IRB Committee related to a study, activity, or procedure
After submitting the Inquiry Form, you can expect a response from the EU IRB at irb@eastern.edu regarding your request.

To begin the process for an IRB Review for your research project, you can complete the IRB Inquiry form above and/or you can access all forms and procedures in the IRB Brightspace page. You can now self-enroll in the IRB Brightspace page to access all instructions, forms, and helpful content. Forms needed for an IRB Review:
- Completed IRB Review Application Form (Microsoft Word)
- Other forms that may be needed after IRB approval (these forms can be found on the IRB Brightspace page): Amendment Request Form, Extension Request Form, Reporting of Adverse Effects Form
