Statement of Financial Support Form- International students who apply for admissions to Eastern University must also submit the financial documentation for the purpose of I-20 issuance.  "Statement of Financial Support" also required notarization.  Instructions are included in the form.

Statement of Expenses- Learn about the tuition and fee costs for Eastern University, Palmer Seminary and Esperanza College programs.

Transfer Form- International students who are holding I-20 form issued by another school must fill this transfer request form in order to transfer SEVIS record from former school to Eastern University.

Health Forms- These are manditory health forms for all incoming, first year, transfer and international students who will be attending the St. Davids campus. Please visit Eastern University's Health Center Website to learn more about Health and Insurance requirements.

F-2 Sponsor Form- International Students who wish to bring their spouse or children to accompany them while studying at Eastern University must fill out this F2 sponsor form in order to receive the I-20 form with dependent(s).
